Directions: GPS ADDRESS- 500 ZUCKSVILLE ROAD FORKS TWP.18040 PHONE # (610) 250-2260 DRIVING DIRECTIONS-Pocono Area: Rte 33 South. Take the PA-191 exit for Stockertown/Nazareth. Turn left at end of the ramp and go over Rte 33 to the Stop Light. Make a Right at the Stop light (Main Street). Main Street becomes E. Center St which becomes Sullivan Trail(4 miles). At traffic light, turn right on to Zucksville Rd (Keystone Nazareth Bank on the Corner). Quick left into the Forks Community center. From Rte 22 East: Take the exit toward PA-611/ 4TH STREET (Easton). At the end of the ramp, Turn SLIGHT LEFT onto PA-248/ BUSHKILL ST. Turn LEFT onto N 3RD ST. N 3RD ST becomes COLLEGE AVE (Go up Hill towards Lafayette College). At Top of the hill, Turn LEFT onto CATTELL ST. Turn SLIGHT LEFT onto KNOX AVE. KNOX AVE becomes SULLIVAN TRL. At the 5th traffic light make a LEFT on Zucksville Rd (Keystone Nazareth Bank on the Corner). Quick left into the Forks Community center.
